case 4

          44 years old married female having 3 children.
C/o:-

  • Swollen painful knees along with painful left ankle bone.
  • Constant headache since two days, afternoon aggravation.
  • Lassitude.
  • Tonsils operated 10 years back.
  • Bulging in centre of chest from gas inhaling etc but not from food.
  • Mild diabetes but desire sweets.
  • No good sleep, only dozing & unrefreshing. sleepy at 2.30 pm.
  • Hot, wants fan.
  • Menses early in 27 days.
  • Likes lemon pickles, but not tamarind. likes curds, averse salt.
  • Wakes at 4.30 or 5 am.
  • Loss of sleep aggravates headache.
  • Tastelessness and poor appetite.
  • Bathes warm, drinks lukewarm water.
  • General aversion to move.
  • Thyroid stimulating hormone tested high (7.7 against the normal range of 0.3 to 5.5)


Does these details are enough to initiate treatment?

  • If NO, what should be further questions / investigations? 
  • If YES, what should be further management?

    1. Causticum is having confusion in the head, as if it were compressed. But it is mainly in the morning. here, we have aggravation in afternoon.

      In causticum there is arthritic and rheumatic pains, especially in the limbs which is similar to our case.

      As per my knowledge causticum doesn't have desire for lemon nor aversion for salt in contrary it has desire for salt.

    1. In Rhus tox, there is stupefying feeling in the head with tingling sensation along with pain in limbs which ameliorates with motion. Here we have general aversion to move.

      There is desire for sweets, sour food, I am not aware about aversion of salt.

      It is hot, and our patient is too.

      It is having swelling, stiffness, and paralyzed sensations in joints, specially which is either from sprains, over-lifting, or over stretching. Here, we do not know exact cause of joint problem.

      In Rhus tox there is general aggravation at night, headache aggravates at morning. here we have aggravation at afternoon.

  3. For knee pain initially at least x-ray is required.

    For headache you can start with any of those analgesics. Try to find out predisposing factors if there is any and treat accordingly.

    1. For sure x-ray will be helpful to know cause of swollen painful knees.

      As well, properly administered analgesics will give quick relief from headache.

  4. I think elimination is not given the due importance it actually deserves.
    DrKarnav is right, rhus is eliminated bcoz of the opposite modality. and also opposite thermals.
    but there is diabetes too. we have so many remedies listed under the rubric urine, sugar- it is easy to select a remedy from that rubric which matches all other remedies. hence I think of either ferrum or China ( both are complementary too) which appear in all the major rubrics and no definite eliminating symptoms are present
